چکیده:
سرطان پدیدهای اجتماعی است. واقعیت اجتماعی سرطان متکثر و برخاسته از فهم و تجربة افراد درگیر با آن است. این پژوهش باهدف فهم معنای سرطان در ذهنیت افراد، بر پایة 43 مصاحبة عمیق و نیم ساختیافته (10 بیمار سرطانی، 10 نفر از اعضای خانواده و 23 نفر از اعضای گروه درمان پرستار، پزشک سرطانشناسی و روانپزشک و روانشناس)، در مرکز اصلی درمان سرطان شهر اصفهان نوشتهشده است. رویکرد پژوهش پدیدارشناسی و در ترکیب با جامعهشناسی تفهمی، روش پژوهش مطالعة کیفی اساسی، شیوة نمونهگیری ترکیبی از نمونهگیری هدفمند و مکمل و ابزار تحلیل، تحلیل مضمونی است. مطالعه روایتهای افراد نشاندهندة پنج مضمون پذیرش سرطان از سوژگی تا تغافل، ابژگی بدن بیمار و بازتعریف نظم زندگی روزمره، دوگانة خانه/ بیمارستان و درهمریختگی فضاها و روابط اجتماعی بیمار، خود بیگانگی بیمار ذیل اقتدار دیگری پزشک و بیمار بهمثابه کلیتی یکپارچه در الگوی مراقبت پرستاری است. بهطورکلی، هرچند روایت ایرانی از سرطان خاص بودگیهای خود را دارد، اما بافهم جهانی از سرطان سازگار بوده که در آن، برساخت میانذهنیت غالب پیرامون سرطان، بیش از آنکه از علم پزشکی و اقتدار آن متأثر باشد از تصویر سرطان در فهم عامه تأثیر میپذیرد که بازتولیدکنندة انگارة بنیادین سرطان بهمثابه نیستی و مرگ است.
Cancer is a social phenomenon with plural significations that are created by the comprehending subjects. Therefore, cancer is best comprehended based on the experience of all people engaged in it other than the patient alone. This research comprehends cancer in the experience and subjectivity of various people by conducting 43 in-depth, semi-structured interviews with 10 cancer patients, 10 family members, and 23 healthcare staff in Isfahan’s major cancer treatment center. The approach of the study is phenomenology accommodated by interpretive sociology, the method is basic qualitative research, the sampling techniques are purposive and complementary, and the analytical technique is thematic analysis. The findings take us to five major themes: dealing with cancer from laxity to subjectivity, patient’s bodily objectivity in redefining the daily life order, the binary of home/hospital and the patient’s spatial and social bewilderment, patient’s alienation under the physician’s other’s authority, and patient as an integrated whole in the nursing care system. Overall, while the Iranian narrative has its own particularities, it complies with universal intersubjective comprehension of cancer in which the dominant intersubjective construct about cancer is rather shaped by the popular comprehensions (reconstructing the basic idea of cancer as death and mortality) than the physicians’ authority.
