چکیده:
مقدمه: بیماران مبتلا به افسردگی و اضطراب همایند، سطح پیچیدهتر و بالاتری از آسیب روانی را نسبت به مبتلایان بدون همایندی تجربه میکنند. هدف: هدف مطالعه حاضر بررسی اثربخشی طرحواره درمانی هیجانی بر عدم تحمل بلاتکلیفی و دشواری تنظیم هیجان افراد مبتلا به افسردگی و اضطراب همایند بود. روش: روش تحقیق حاضر نیمه تجربی و طرح آن پیشآزمون- پسآزمون با گروه کنترل بود. از میان بیماران مبتلا به افسردگی و اضطراب همایند مراجعهکننده به مراکز مشاوره شهر اصفهان در سال ۱۳۹۹، تعداد ۳۲ نفر از بانوان از طریق مصاحبه بالینی به صورت در دسترس انتخاب و به طور تصادفی در دو گروه طرحواره درمانی هیجانی (۱۶ نفر) و گروه کنترل (۱۶ نفر) گمارده شدند. ابزارها شامل مقیاس عدم تحمل بلاتکلیفی (۱۹۹۴) و مقیاس دشواری تنظیم هیجان (۲۰۰۴) بود. طرحواره درمانی هیجانی در ۱۰ جلسه هفتگی ۹۰ دقیقهای اعمال شد. دادهها از طریق تحلیل واریانس با اندازهگیری مکرر و آزمون تعقیبی بونفرونی با نرمافزار SPSS نسخه ۲۲ تجزیه و تحلیل شدند. یافتهها: یافتهها حاکی از این بود که طرحواره درمانی هیجانی اثربخشی درونگروهی و بینگروهی نیرومندی را بر عدم تحمل بلاتکلیفی و دشواری تنظیم هیجان داشت (۰/۰۱>P). اثربخشی این مداخله تا مرحله پیگیری ماندگار بود. میزان اندازه اثر برای عدم تحمل بلاتکلیفی و دشواری در تنظیم هیجان به ترتیب ۰/۳۲۴ و ۰/۴۳۰ بود (۰/۰۱>P). نتیجهگیری: طرحواره درمانی هیجانی میتواند به عنوان یک درمان یکپارچه عوامل فراتشخیصی اختلال افسردگی و اضطراب همایند را بهبود بخشد. با اثبات کارایی این مداخله در مطالعات متعدد، میتوان از آن به عنوان خط اول درمان افسردگی و اضطراب همایند استفاده کرد.
Introduction: Patients with depression and comorbid anxiety experience more complicated and higher levels of psychological harm than the ones without comorbidity.
Aim: The purpose of this study was to investigate the effectiveness of emotional schema therapy on the intolerance of uncertainty and the difficulty of emotion regulation in people suffering from comorbid depression and anxiety.
Method: This study was a quasi-experimental research, applying a pre-test and post-test control group design. After interviewing a group of patients with depression and comorbid anxiety who attended counseling centers in Isfahan in 2019, 32 women participants were chosen. Then they were randomly assigned to two groups, including an experimental group and a control group. Each group consisted of 16 participants. The Intolerance of Uncertainty Scale (1994) and the Emotion Regulation Difficulty Scale (2004) were used as the instruments of the study. The experimental group went through emotional schema therapy, which lasted for ten 90-minute sessions. The data analysis was performed through repeated measures ANOVA and Bonferroni's post hoc test, using SPSS-22.
Results: The results of the study indicated that emotional schema therapy had a significant effect on uncertainty intolerance and emotion regulation difficulty in both intra- and inter-group status (P
Conclusion: Emotional schema therapy as a unified treatment can improve trans-diagnostic factors of depression and comorbid anxiety. If proven in various studies in terms of applicability, this intervention can be pioneering in the treatment of depression and comorbid anxiety.
