چکیده:
زمینه: تحقیقات نشان میدهد بیماریهای قلب و عروق علت اصلی مرگومیر در ایران است و بیماران پس از جراحی با مشکلات روانشناختی مانند افسردگی، اضطراب و تنیدگی مواجه میشوند. روش شمارش پیشرونده به عنوان یک مواجهه ذهنی و حساسیتزدایی از خاطرات آسیبزا منجر به کاهش اضطراب و تقویت حس تسلط بر خاطرات غیرمنتظره کارآمد بوده، اما در زمینه کاربرد این روش در جامعه مورد تحقیق شکاف تحقیقاتی وجود دارد.
هدف: پژوهش حاضر با هدف بررسی اثربخشی درمان شمارش پیشرونده بر نشانههای هیجانی (افسردگی، اضطراب و تنیدگی) در بیماران CABG انجام شد.
روش: در این پژوهش طرح پیشآزمون - پسآزمون با گروه گواه استفاده شد. جامعه آماری این مطالعه بیمارانی بودند که ۴ الی ۶ هفته بعد از CABG به کلینیک بازتوانی مرکز قلب تهران ارجاع شده بودند. از بین این افراد، ۲۴ نفر به روش در دسترس انتخاب شدند. این افراد پس از کسب رضایت، به صورت تصادفی در دو گروه مداخله و گواه قرار گرفتند. سنجشها با استفاده از DASS-21 (سامانی و جوکار، 1386) و IES-R (پناغی، حکیم شوشتری و عطاریمقدم، 1385) در سه مقطع پیشآزمون، پسآزمون و پیگیری در هر دو گروه انجام گرفت.
یافتهها: نتایج تحلیل آماری نشان داد که روش درمان شمارش پیشرونده در کاهش معنادار میزان اضطراب و تنیدگی در زمان پیگیری در گروه مداخله موثر بوده است (0/05 =α). این روش تفاوت معناداری بین دو گروه در میزان افسردگی در زمان پسآزمون و پیگیری ایجاد نکرده است و میزان افسردگی در دوره پیگیری به میزان زیادی توسط این مقادیر در مقطع پیشآزمون تعیین میشود (0/05 =α).
نتیجهگیری: مداخله شمارش پیشرونده مبتنی بر آگاهی از ضربه احتمالا تحت تاثیر فهم اثرات آسیب بر زندگی فرد در راستای بهبود اثرات آسیب بر زندگی و کاهش احتمال مجددا در معرض آسیب قرار گرفتن تاثیر دارد، انجام مطالعات با نمونههای بیشتر توصیه میگردد
Background: Former studies have demonstrated that coronary artery disease is a major cause of death and in Iran is the top cause of death and causes 45 percent of fatalities. After coronary artery bypass surgery (CABG) patients face psychological problems such as depression, anxiety, and stress. Progressive counting method is an effectiveness method in providing mental exposure and desensitize traumatic memories to lower anxiety and enhance a sense of mastery over spontaneous memories. But there is lack of research on the progressive counting method in Iran.
Aims: This study aimed to evaluate the efficacy of PC method on emotional symptoms (depression, anxiety & stress), which exist in CABG patients according to the literature.
Methods: In this quasi-experimental research, we used a pre-test-post-test design. Patients included those who were referred to the Rehabilitation Clinic of Tehran Heart Center within a 4 to 6-week period after coronary artery bypass surgery. Among them, 24 patients were selected. Having consented to participate in the study, all 24 participants were randomly placed in intervention and control groups. DASS-21 and IES-R were used for measurement. Assessments were performed in three stages pre-test, post-test, & follow-up in both groups.
Results: Progressive counting method significantly reduced anxiety & stress symptoms in the intervention group (α=0.05). This method did not make a significant difference in the rate of depression. The amount of depression during the follow-up period is largely determined by these values during the pre-test period (α=0.05).
Conclusion: Progressive counting method probably influences the perception of the impacts of the trauma on the life of the individual and treats the implications of the trauma on the patient's life and decreases the probability of re-traumatization. It is recommended to study with more samples.
