چکیده:
مقدمه: هدف این پژوهش بررسی تأثیر روان درمانی پویشی بلند مدت با رویکرد روابط موضوعی کرنبرگ بر ساختار شخصیت بیماران مبتلا به اختلال شخصیت وابسته بود. روش: 30 مراجع مبتلا به اختلال شخصیت وابسته از بین زنان مراجعه کننده به مراکز درمانی تهران بر اساس معیارهای تشخیصیDSM5 و ملاک های ورود و خروج انتخاب شدند؛ آزمودنیها به روش نمونه گیری هدفمند انتخاب و به صورت تصادفی به دو گروه آزمایش و کنترل گمارش شدند. رضایت آگاهانه از آزمودنی ها اخذ شد و گروه آزمایش به مدت 6 ماه و دو جلسه در هفته تحت درمان روان پویشی بلند مدت قرار گرفتند، گروه کنترل نیز به منظور رعایت اخلاق پژوهش در نوبت رواندرمانی قرار گرفت. مصاحبه ساختار یافته برای ارزیابی سازمان شخصیت (STIPO) در ابتدا از هر دو گروه گرفته شد. علاوه بر آن پس از 6 ماه و پس از پایان جلسات درمانی نیز مجدداً تکرار گردید. یافتهها: نتایج نشان داد گروه آزمایش کاهش معناداری در میانگین ابعاد ساختاری خشکی شخصیت، هویت و مکانیزم های دفاعی پس از 6 ماه از درمان و در ابعاد ساختاری خشکی شخصیت، هویت، مکانیزم های دفاعی، روابط موضوعی و اخلاقیات پس از اتمام درمان نسبت به گروه کنترل نشان داد. نتیجهگیری: برحسب نتایج حاصل از این پژوهش مداخله روان درمانی پویشی بلند مدت با رویکرد روابط موضوعی کرنبرگ بر تغییر ساختار شخصیت مبتلایان به اختلال شخصیت وابسته اثرگذار بود. به متخصصان بالینی در کار با بیماران مبتلا به اختلال شخصیت وابسته استفاده از این درمان توصیه می شود.
Introduction: The present research aimed at investigating the effect of long-term dynamic psychotherapy with Kernberg’s object relations approach on the personality structure of patients with Dependent Personality Disorder (DPD). Method: 30 clients with DPD were selected among the individuals referred to
for psychotherapy, and based on DSM-5 diagnostic criteria they met the inclusion criteria. The subjects were selected randomly among referred patients to clinics and randomly divided into two experimental and control groups. The informed consent was obtained from the subjects and the experimental group received DPHP sessions for 12 months in a twice weekly session framework. The control group was assigned to psychotherapy in order to observe the ethics of the study. The Semi-Structured Interview of the Personality Organization (STIPO) was initially taken from both groups and was repeated again after six months and the end of the treatment sessions. Results: The results indicated that the experimental group had a significant reduction compared to the control group in the structural dimensions of defence mechanisms, personality rigidity and identity after 6 months of treatment and in the structural dimensions of object relations, personality rigidity, identity, morality and defence mechanisms at the end of the treatment. Conclusion: According to the results of this research, long-term psychotherapy intervention with the Kernberg's object relations approach was found to be effective in changing the personality traits of patients with dependent personality disorder and clinical specialists are recommended to use this treatment for patients with DPD.
