Abstract:
اهداف سرعت زیاد رشد جمعیت سالمندان موجب بروز مشکلات بیشتری نظیر کاهش استقلال میشود. با توجه به اهمیت انجام مستقل فعالیتهای روزمره زندگی در سالمندان، این مطالعه با هدف تعیین ارتباط بین حمایت اجتماعی، وضعیت شناختی و علائم افسردگی با فعالیتهای روزمره زندگی در سالمندان شهر امیرکلا انجام شد.
مواد و روش ها این مطالعه آزمایش-کنترلی همسانشده، قسمتی از فاز اول طرح کوهورت سالمندان شهر امیرکلا است که بر روی کلیه افراد 60 سال و بالاتر شهر امیرکلا انجام شد. برای گردآوری دادهها از پرسشنامههای مختلف شامل مشخصات جمعیتشناختی، ارزیابی مختصر ذهنی، شاخص حمایت اجتماعی دوک، افسردگی سالمندان، فعالیتهای روزانه زندگی و فعالیتهای ابزاری روزانه زندگی استفاده شد. تجزیه و تحلیل آماری متغیرها با استفاده از آزمونهای آماری توصیفی (فراوانی، میانگین و انحرافمعیار) و تحلیلی (تی مستقل، رگرسیون لجستیک شرطی) با کمک نسخه 21 نرمافزار SPSS انجام شد.
یافته ها در این مطالعه، میانگین وضعیت شناختی در سالمندان بدون ناتوانی در انجام فعالیتهای روزمره زندگی بهطور معناداری بیشتر از سالمندان با ناتوانی در انجام فعالیتهای روزمره زندگی بود (0/001=P). همچنین در سالمندان با ناتوانی در انجام فعالیتهای روزمره زندگی، میانگین نمره افسردگی و وجود بیماری بهطور معناداری بیشتر و میانگین حمایت اجتماعی و توانایی برای انجام فعالیتهای ابزاری روزانه زندگی بهطور معناداری کمتر بود (0/001=P). یافتههای آنالیز رگرسیون لجستیک شرطی تعدیلشده نشان داد در سالمندان دارای اختلال شناختی، بیماریهای مزمن و نابرخوردار از حمایت اجتماعی، شانس ناتوان شدن در انجام فعالیتهای روزمره زندگی بیشتر است (05/P
نتیجه گیری وجود اختلال شناختی و ابتلا به بیماریهای مزمن همزمان در سالمندان، بیشترین نقش را در ایجاد ناتوانی در انجام فعالیتهای روزمره زندگی دارد. این در حالی است که افزایش حمایت اجتماعی در افزایش توانایی انجام فعالیتهای روزمره زندگی سالمندان مؤثر بوده است. بنابراین غربالگری سالانه سالمندان از نظر وضعیت سلامت و ابتلا به بیماریهای مزمن، وجود وضعیت شناختی و بهبود و گسترش شبکه حمایت اجتماعی سالمندان میتواند نقش مؤثری در پیشگیری از ناتوانی در انجام فعالیتهای روزمره زندگی داشته باشد.
Objectives: The rapid growth of elderly population has led to more problems such as declining independence. Due to the importance of doing independent daily living activities in the elderly, this study was conducted to determine the relationship between social support, cognitive status and depressive symptoms with daily activities in the elderly in Amirkola.
Methods & Materials: This mached case-control study is part of the first phase of the cohort project for the elderly in Amirkola, which was conducteded on all people aged 60 years and more in Amirkola. To collect the data, various questionnaires including demographic characteristics, Mental Brief Assessment (MMSE), Duke Social Support Index (DSSI), Elderly Depression Index (GDS), Activity of Daily Living (ADL) and Instrumental Activity of Daily Living (IADL) were used. Results: In this study, the mean of cognitive status in the elderly without functional disability was significantly higher than the elderly with functional disability (p = 0.001). Also, in the elderly with functional disabilities, the mean score of depression and the presence of disease was significantly higher and the mean of social support and ability to perform activity of daily living were significantly lower (p = 0.001).Findings of adjusted conditional logistic regression analysis showed that the elderly with cognitive impairment, chronic diseases and lack of social support are more likely to be disable in their activity of daily living(p
Conclusions: The presence of cognitive impairment and concomitant chronic diseases in the elderly has played the greatest role in disability of elderly in activities of daily living. However, increasing social support was effective in increasing the ability to perform activity of daily living of the elderly. Therefore, annual screening of the elderly in terms health status and chronic diseases and cognitive status and improvement and expansion of the social support network for the elderly can play an effective role in preventing functional disabilities.
